But there’s a real, specific physiological mechanism behind why certain breathing patterns reduce stress fast — and a 2023 clinical trial gives a clearer answer than most wellness content on which techniques actually work best.

Why breathing affects your stress levels at all

Breathing is unusual among bodily functions in that it runs automatically but can also be consciously controlled. That dual control is the mechanism relaxation techniques exploit: slowing and lengthening your breath, particularly the exhale, activates the parasympathetic (“rest and digest”) branch of the nervous system and reduces heart rate, which the brain reads as a signal that the immediate threat has passed. Short, shallow breathing does the opposite — it’s part of what keeps the sympathetic (“fight or flight”) response switched on.

The NHS’s standard technique

NHS guidance recommends a simple, low-effort exercise for stress: breathe in gently through the nose for a count of 5, then out through the mouth for a count of 5, repeated for at least five minutes. You don’t need to force reaching the count of 5 at first — comfort matters more than precision. It can be done sitting, standing or lying down, and works as both a daily practice and an in-the-moment tool when stress is building.

What a real Stanford trial found

In 2023, a randomised controlled trial published in Cell Reports Medicine by researchers at Stanford University (Balban, Neri, Kogon, Zeitzer, Spiegel and Huberman) directly compared different daily 5-minute breathing exercises against mindfulness meditation over a month, in 108 participants split across four groups: cyclic hyperventilation, exhale-focused “cyclic sighing,” box breathing, and a mindfulness meditation control.

The result: cyclic sighing — breathing with an emphasis on long, slow exhales — produced a significantly greater improvement in mood and a significantly greater reduction in resting respiratory rate than mindfulness meditation did, over the same daily time commitment. This matters because it’s one of the few head-to-head trials comparing specific breathing patterns rather than just breathing versus nothing.

The physiological sigh, step by step

The specific pattern used in the “cyclic sighing” arm of that trial — sometimes called the physiological sigh — is:

Take a full inhale through the nose

Without exhaling, take a second, shorter inhale through the nose to top up your lungs

Follow with one long, slow exhale through the mouth, letting the exhale run longer than the two inhales combined

One full cycle takes roughly 30 seconds. It’s designed as a fast-acting tool for an acute stressful moment (before a difficult conversation, mid-panic, after a stressful email) rather than a long meditative practice, which is part of what makes it practical for everyday use.

Box breathing, for a different situation

Box breathing (also tested in the same trial, though it performed less well than cyclic sighing for mood improvement) follows an even four-count pattern: inhale for 4, hold for 4, exhale for 4, hold for 4. It’s widely used because the structure itself gives the mind something concrete to focus on, which can help when anxious thoughts are racing rather than when you just need a fast reset.

Which technique to use when

Feeling acutely stressed right now (before a meeting, after bad news): one to three rounds of the physiological sigh.

Building a daily calming habit: five minutes of NHS-style 5-count breathing or cyclic sighing, done consistently, which the Stanford trial suggests may outperform an equivalent block of mindfulness meditation for mood specifically.

Racing, intrusive thoughts: box breathing, where the counting structure gives the mind an anchor.

None of these techniques are a substitute for treatment of a diagnosed anxiety disorder, but as a free, evidence-backed, immediate tool for everyday stress, breathing technique genuinely is one of the highest-value places to start — not because it’s trendy, but because there’s now a real controlled trial behind it.

Sources

NHS – Breathing exercises for stress

PubMed – Balban et al., “Brief structured respiration practices enhance mood and reduce physiological arousal,” Cell Reports Medicine, 2023

Cell Reports Medicine – Full study text

“Just breathe” is easy advice to dismiss, because it’s usually given without any explanation of why it would help.
